Membership
Simple, Transparent Membership Pricing
Your membership covers primary care visits and direct access to Dr. Willis. Medications, labs, imaging, and procedures may have separate costs, as outlined below.
One simple monthly membership
Age-based pricing, per person.

- Ages 25–39$109/month
- Ages 40–64$129/month
- Ages 65–99$150/month
One-time enrollment fee: $100 per person.
HSA & FSA payments
HSA funds can pay for qualifying direct primary care memberships. Confirm that your COS DPC arrangement qualifies before using HSA funds. FSA reimbursement is not automatic; check with your plan administrator before paying.
Medicare and TRICARE beneficiaries will first be placed on a waitlist.
We are unable to see Medicaid recipients at this time.
Family & community discounts
- 5% discount for every family member.
- 10% discount for full-time students, military, first responders, and educators.

Frequently Asked Questions
Do I still need health insurance if I have a COS DPC membership?
A COS DPC membership is not health insurance. It does not replace coverage for hospitalization, emergency room visits, surgery, specialist care, or other services outside the practice. Consider maintaining traditional health insurance or a health share to help cover care that may require hospitalization or specialty services not offered by COS DPC.
What happens if I need care outside of what’s included in my membership?
Services outside your membership may have separate charges. Ask the practice about the expected cost and how any outside care will be coordinated before proceeding.
How do labs and medications work with the wholesale/discounted pricing?
Medications dispensed through the office are offered at wholesale cost, and discounted labs and imaging are available. These are separate from your monthly membership fee. Ask us about pricing for the specific medication or test you need.
