Membership
Simple, transparent pricing.
Primary care visits with no visit copays and direct access to Dr. Willis. Labs, medications, imaging, and procedures may cost extra. Membership is not health insurance.
Family members receive a 5% discount. A 10% discount is available for full-time students, military, first responders, and educators. Contact us to confirm eligibility and your final cost.
Medicare and TRICARE beneficiaries are placed on a waitlist. We are currently unable to see Medicaid recipients.
One simple monthly membership
Age-based pricing, per person.
- Ages 17–24$89/month
- Ages 25–39$109/month
- Ages 40–64$129/month
- Ages 65–99$150/month
One-time enrollment fee: $100 per person.

Dr. Cornelia J. Willis, MD, MPH
Board-certified Internal Medicine physician · U.S. Army veteran
Dr. Willis earned her medical degree at the University of Vermont and her Master of Public Health at George Washington University. She completed her Internal Medicine residency at Madigan Army Medical Center.
At COS DPC, she provides relationship-based care with time to understand your health history and concerns.

Is this a membership or a pay-per-visit practice?
COS DPC offers membership-based primary care. Monthly pricing is based on age, with a one-time enrollment fee. If you are looking for a single visit rather than ongoing care, call the practice to discuss your needs before making plans.
Are all of these services included in the monthly price?
Membership includes primary care visits with no visit copays. Labs, medications, imaging, and procedures may have separate charges. Ask about the specific service you need and review the pricing page for membership terms.
Can I join if I have Medicare, TRICARE, or Medicaid?
The published enrollment policy places Medicare and TRICARE beneficiaries on a waitlist. The practice is currently unable to see Medicaid recipients. Contact the practice to confirm eligibility before enrolling.
Does membership replace health insurance?
No. Membership does not replace insurance for hospital care, emergency visits, surgery, specialists, or other care outside the practice.
